To request a transcript, please follow the directions below:

  • Download the Transcript Request Form (PDF).
  • Fill out the form.
  • Fax the form to 816.268.5500 or mail the form to:
          Nazarene Theological Seminary
          Office of the Registrar
          1700 E. Meyer Blvd.
          Kansas City, MO 64131

Important Information:

  • Please sign the form, otherwise the request cannot be completed.
  • There is no fee for transcript requests.
  • Each week, requests must be in by 4:30pm on Thursday, because transcripts are mailed every Friday (except holidays).
